Thanks for signing up! I’m Glein, Dr. Alissa Citron's assistant, and I’m really excited to welcome you to Kite Dream Care. Dr. Baines is passionate about providing better, more personalized care, and we’re so happy you’ve joined us on this journey.

Over the next few days, I’ll be sending you some emails with important information. This message is worth reading and will also be emailed to you, while the next few are more for reference—you can just skim the bold sections and save them for later.

What’s Next?

Take a moment to think about what you’d like to get out of this program! Dr. Citron will help you define your goals—whether they’re traditional medical goals, life goals, brain goals, or even relationship goals.

Today, I’ll be sending you three forms:
1️⃣ Logistics Form – Your preferred way to communicate (text, phone, email), closest pharmacy, labs/imaging centers, and whether you prefer in-office or telemedicine visits.
2️⃣ Patient Medical History Form – A basic overview of your health history.
3️⃣ Goal/Visioning Exercise – A chance to reflect on what you want from your healthcare. If any section feels difficult, just skip it and move on.

Once You Complete the Forms

Schedule an intake appointment (in-person or virtual) to review everything with Dr. Baines. These typically take 30-60 minutes, and there’s no rush to book.

If You See a Specialist, Get Labs, or Imaging

Please make sure they have this new contact information so records don’t get sent to Bloomfield Health or a previous provider. Then, text or call within a day or two so we can confirm we’ve received your results. Dr. Baines can also access Beaumont MyChart if needed.
